Print the attached Contractor Agreement and Contractor Addendum, sign them and send them back with copies of the following documents:

1. Proof of commercial general liability insurance coverage with limits of no less than $1,000,000 per occurrence.
2. Commercial automobile liability insurance with a limit for bodily injury and property damage of not less than $1,000,000 each accident.
3. Proof of Compliance with State Worker’s Compensation Laws (if applicable)
4. State Licenses.
5. W-9
